For Trucker or Motor Carrier Applications
ACORD 125 – Applicant Information Section
For Trucker Motor Carrier risks the first ACORD form you use in putting together an application is the ACORD 125. Enter all basic insured information in the ACORD 125.
Truckers Motor Carrier ACORD form 132 handles the basic underwriting and rating needs for liability and physical damage coverages for trucking or motor carrier operations.
If a transportation or motor truck legal liability exposure exists, the Transportation Section, ACORD 143, may also need to be completed.
Each state has it’s own form 137. You need to go to the “State Specific ACORD Forms” links to find these forms.
Because insurance is regulated on a state by state basis, the ACORD 137 form is where you show what coverages and limits your insured is requesting.
I am showing an image of the North Carolina 137NC form. If you were working with an insured with vehicles titled in Texas you would use the Texas ACORD 137TX, in New York the ACORD 137NY and so on.
ACORD 163 – Driver Information Section if there are more than 6 drivers
The ACORD 163 is where you enter the driver information for additional drivers the ACORD Trucker Motor Carrier form 132 does not have room for. The ACORD 163 form has room for an additional 24 drivers. Attached multiples of this form until you have all the drivers listed.
ACORD 129 – Vehicle Schedule
The Trucker Motor Carrier form 132 has no space for any vehicles. The ACORD 129 Vehicle Schedule is where you enter vehicles to be covered for under the Trucker Motor Carrier policy. You can enter six additional vehicles per form. Keep using additional forms until all vehicles are listed.
If you insured has a legal liability exposure for the cargo he is hauling, teh 143 Inland Marine form needs to be part of you application.
ACORD 194 – Truckers and Motor Carriers Supplemental
This form is intended to supplement ACORD 132 Trucker Motor Carrier Section when state or federal filing action is required and the Regulation section in ACORD 132 is not adequate to provide the company with the necessary filing information.
ACORD 60 or 61 forms for your states specific Un/Underinsured Motorist selection/rejection
Most states require the insured to have the option of specifically waiving parts, or all, of uninsured and underinsured motorist coverages. You should use the form for your specific state to accomplish this.
ACORD 60US – Terrorism Coverage
Your insured has the option of rejecting Terrorism Coverage as a part of the Trucker Motor Carrier application. Use the ACORD 60US if your insured wishes to reject Terrorism Coverage.
ACORD 63 – Fraud (Some states require a state specific form)
You should have your insured sign the General Fraud Statement as a matter of course.
Some states have specific wording for their Fraud Forms. You should use any state specific form that applies instead of the general form.
