ACORD 51 GA (2005/01) – Georgia Fleet Policy Information Card

The Georgia Fleet Policy Information Card was created in response to Georgia regulations. The main differences between the generic ACORD Automobile Insurance Card, ACORD 50, and the Georgia card are:

* Company number is not required.
* Agency/Company issuing card is not required.
* Notice on reverse side of card states “”Keep this card in your motor vehicle while in operation””.

Insurance Company Name

Name of the insurance company providing the coverage

Policy Number

Number exactly as it appears on the policy, including prefix and suffix symbols.

Original Issue Date

Date the policy was originally issued.

Expiration Date

Enter the expiration date shown in the policy.

Name of Insured

Name and mailing address of the insured as found on the declarations page of the policy.
