TITLE ACORD 50 (2007/02) Automobile InsuranceID Card The ACORD Automobile Insurance Identification Card (ACORD 50) is accepted in the majority of states that require the insured to carry/produce upon demand proof of insurance. The states where ACORD 50 is not acceptable are: – Arkansas – California – Delaware – Florida – Georgia – Hawaii – Iowa – Kentucky – Louisiana – Michigan – Mississippi – Missouri – New Jersey – Nevada – New York * – Oklahoma – Pennsylvania – Texas – West Virginia * Effective 1/1/02, the New York ID card must be issued by means of an encrypted bar coded software program available only from insurance companies. ACORD does not provide the ID card.
INSURANCE IDENTIFICATION CARD State Full name of state as shown in the address on the policy.
INSURANCE IDENTIFICATION CARD Blank Space Certain states require special provisions to be added to the Auto ID card exactly below the form title. Use this space for any additional required text in these states.
INSURANCE IDENTIFICATION CARD Company Number The Company Number that must be shown on the card is the NAIC number for the individual company issuing the policy.
INSURANCE IDENTIFICATION CARD Company Name of the applicable insurance company. Use the actual name of the company within the group to which the policy has been issued. Do not use group names.
INSURANCE IDENTIFICATION CARD Policy Number Number exactly as it appears on the policy, including prefix and suffix symbols.
INSURANCE IDENTIFICATION CARD Effective Date Enter the effective date shown in the policy.
INSURANCE IDENTIFICATION CARD Expiration Date Enter the expiration date shown in the policy.
INSURANCE IDENTIFICATION CARD Year Year of the motor vehicle.
INSURANCE IDENTIFICATION CARD Make/Model Make and Model of the motor vehicle.
INSURANCE Vehicle Identification Number Vehicle Identification Number of the motor vehicle.
INSURANCE IDENTIFICATION CARD Agency/Company Issuing Card Name and address of the issuing agency or company.
INSURANCE IDENTIFICATION CARD Insured Name and mailing address of the insured as found on the declarations page of the policy.

TRAVEL TO CANADA OR MEXICO
Contact your insurance company when a US insured needs proof of coverage for travel to Canada or Mexico
Special Provisions/State Exceptions to ACORD ID CARD (ACORD 50).
Alabama Add the following wording: “Policy provides the minimum insurance prescribed by law.” ACORD 50 AL contains this wording.
Arizona The COMPANY NUMBER in this state is the number assigned to the company by the Arizona DOT. ACORD 50 AZ contains this provision.
Colorado Must display the coverage required by law; BI, PD, PIP (effective 7/1/03, BI and PD only). Limits need not be stated. ACORD 50 CO contains the wording, “BI and PD Coverage provided”.
Connecticut Add the following wording: “Connecticut Insurance Card issued pursuant to Connecticut Law.” This text should appear under the title of the ID card. Issue in duplicate. ACORD 50 CT meets the criteria referenced above.
Idaho The document may, but is not required to be entitled “Certificate ofLiability Insurance” or “Liability Insurance Identification Card”. The words “State of Idaho” may be added to the title at the insurer’s option, but the words “State of Idaho” are not required. ACORD 50 ID is entitled State of Idaho Liability Insurance Identification Card.
Illinois Add the following wording: “Examine policy exclusions carefully. This form does not constitute any part of your insurance policy.” Excluded drivers should be shown on the ID card as excluded from coverage. ACORD 50 IL meets the criteria referenced above.
Indiana Financial Responsibility filing only. Either of the following must be shown in the space for AGENCY/COMPANY ISSUING CARD: * Name,address and phone number of insurer, or * Name of insurer and name, address and phone number ofinsurance agency. ACORD 50 IN meets the second criteria referenced above.
Kansas Cannot be used by those vehicles subject to the State Corporation Commission.
Maine Title should be “Maine MotorVehicle Insurance Identification Card.” * The following should also be added to the card (may be added to the reverse side): “The policy provides the minimum insurance required by law.” ACORD 50 ME meets the criteria referenced above.
Minnesota Plain language summary of sections 169.791, 169.793 and 169.797 of Minnesota law must accompany the card, but does not have to be printed on card. The following language is advisory and can be modified: * “Every driver shall have in his or her possession while operating a motor vehicle, and shall produce on demand proof of insurance covering the vehicle being operated. Failure to produce the required proof of insurance can result in a misdemeanor conviction.” * “It is unlawful for any person to display, cause or permit the display of, or have in possession proof of insurance that is fictitious or fraudulent.” * “In addition to criminal penalties, any person convicted of a misdemeanor because of any of the above is subject to drivers license revocation, and a fine of not less that $200.”
Nebraska Title of the form is Nebraska Auto Liability Insurance Identification Card. ACORD 50 NE meets the criteria referenced above.
Rhode Island Add the following wording: “Policy meets Rhode Island limits.”
South Carolina Add the following wording: “Coverage meets SC minimum financial responsibility requirements” to the front of the card. ACORD 50 SC meets the criteria referenced above.
South Dakota Issue in duplicate. Add the following wording: “Coverage Provided by this Policy Meets the Minimum Liability Limits Prescribed by Law.” ACORD 50 SD meets the criteria referenced above.
Tennessee Add the following wording: “An Insurance Policy Has Been Issued that Meets Requirements of Tennessee Financial Responsibility Law of 1977.” ACORD 50 TN meets the criteria referenced above.
Utah Excluded drivers should be shown on the ID card as excluded from coverage. ACORD 50 UT meets the criteria referenced above.
Vermont Title: “Vermont Automobile Insurance Identification Card.” Add the following wording: “Policy Provides the Minimum Insurance Prescribed by Law.” ACORD 50 VT meets the criteria referenced above.
