ACORD 193 (2001/02)Open Cargo Section

Use this form to provide basic information for marine coverage involving an open cargo policy.

Individual company manuals should be consulted for unique underwriting, rating, and other information required by specific companies.

This form is designed to be used in conjunction with the Commercial Insurance Application – Applicant Information Section, ACORD 125.

INTEREST
Check the appropriate box to indicate the interest of the applicant in the property.

CONVEYANCE USED
Describe the method of transportation that will be used to move the property.

OPERATIONS
Describe the property that will be shipped, the locations of the points of origin and the locations of the points of destination.

GENERAL INFORMATION
Provide the average value, type of packing used, annual gross sales, and the coverage form desired.

COVERAGES/ADDITIONAL COVERAGES
Provide coverage information required by the insurance company.

EXCLUSIONS
Describe the applicable exclusions required.
