How to Use Fill In and Print Forms
1-First, your computer opens and loads Acrobat Reader. Depending on the speed of your computer, this may take 10 or even 20 seconds.
2-Acrobat may load behind this screen. If you do not see a new page load, look at the bottom of your screen at the taskbar. You will see the Acrobat logo with a file name in it. Click on that box on the Taskbar to bring the form to the front of your screen.
3-Move your cursor around the screen. Notice how the cursor changes as you pass over different areas. Your cursor changes when it is over an area you can fill out. Click in a box and start typing.
4-When your cursor is over a check box, simply click the left mouse button to check that box.
5-Some check box groups are mutually exclusive. For example, you can not have both yes and no as an answer. For these boxes, check yes and then check no, the yes response will be cleared when you check no.
6-Some information is needed as numbers. Examples are coverage amounts or premium amounts. In these fields, if you try to enter anything other than a number, the form beeps at you. (Sorry, I hate that too.)
7-Dates can actually be entered in any way you like. We do not edit this, so be careful to enter the day, month and year as needed.
8-We automatically fill in today's date for you in the field at the top of the form. You can erase this and enter any date you would like.
9-The RESET button clears all the data you have entered. BE CAREFUL with the RESET button. There is no way to get you entries back.
10-The text you enter appears in blue. Checks appear in red. The information that is a permanent part of the form appears in black. You can not change the things that show up in black.
