ACORD 73 (2000/08) - Solid Fuel Questionnaire - Personal Property Supplement


Use this form with all personal property applications, when the property contains a heating appliance that uses solid fuel such as coal or wood, and specific information about such heating appliances is required by the company


SOLID FUEL DEVICE

Provide the name of the manufacturer, brand name, model number, and type of fuel for each device. Complete the balance of this section as applicable.

CHIMNEY/STOVE PIPE/UNIT CLEARANCES

Complete any or all of these sections that apply to the particular device(s) on the property..

FIRE PROTECTION

Indicate the type of fire protection, if any, on the premises.

CLEANING

Answer these questions for all appliances.
