ACORD 72 (2002/12) - Certification of Mobile Home Tie Downs - Supplement to Mobile Home Application

Use this form with ACORD 85, Mobile Home Application, to provide information about the type of tie-down protection provided for the mobile home. Consult your company as to when this supplement is required.

PARK AND MOBILE HOME IDENTIFICATION

Park Name
Provide the name of the mobile home park, if any.

Date Mobile Home "Put In Place."
Provide the date the mobile home was first located at this address.

Year, Make, Model, Serial Number, Length, Width
Provide this information for the mobile home.

TIE DOWNS

If the mobile home is tied down, answer all the questions. Use the EXPLANATION section to provide information not adequately explained by checking the boxes.

MINIMUM TIE DOWN REQUIREMENTS

Information contained on the back of the form is intended to be guidelines for minimum tie-down requirements. Note that installation of tie downs does not assure safe occupancy during severe winds and hurricanes.
