find our Free ACORD forms by SEARCH or FOLDER TREE.
To find a Form
using "Search, click on the "Search" tab. Enter any partial name
of the form you are looking for in the search text box. Click on the Search
button. All forms with a match will display in the results box.

How to
Open the ACORD Form
To open a form
double click the form in the Search box ( or the Folder Tree). You will then
see this form name appear in the larger box on the right hand side of the screen.

Click on the OPEN
button below the larger box the form was moved into. Your local computer will
have to load your Adobe Reader. This may take a moment the first time.

Open More than One Form at a Time
You can highlight and select as many forms as you need and
move them one at a time into the box on the right hand side of the screen. Then
whne you click OPEN, all the forms open at the same time. Here is an example.

Using the Folder Tree
When you click on any folder in the Folder Tree, the folder
expands to show its contents. The folders contain what we hope are fairly obvious
groupings of the ACORD forms.

For example, all certificate of insurance related forms can
be found in the folder titled "Certificates".

Change Request Forms are found in the Changes
folder.

CountryWide forms are forms like the ACORD
125 that is used in every state.

Life Insurance Forms are found in the Life
Folder

Loss Notices and related forms are in the
Loss Folder

State Specific forms - like each state auto
applications - are somewhat different.
The State folder contains a folder for each state. When you
open the specific state folder you find the ACORD forms unique to that state.

Withdrawn forms are retained in the Withdrawn
folder in case you need to use the older version for some reason.
